Applying

If you wish to apply to the Court for a decision you will need to complete the relevant forms depending on the type of order or decision you are asking the court to make. The booklet PDF COP42 Making an application to the Court of Protection (PDF 0.17MB) provides guidance on how to make an application.

The table contained in the COP42 lists the type of form you need to complete for the most common types of application.

Certain applications will require different forms and supporting information, details of which can be found in the relevant court Practice Direction. You download all the practice directions in the practice directions and COP rules page of our site.

You can download copies of the Court forms from the Court of Protection forms page of our site.

Application fee

You will usually need to pay a fee with your application. Details of the fees charged by the court, and the circumstances in which the fees may be waived or reduced, are available from the fees section of our website.
